  • While using insurance can lower out-of-pocket costs, it often comes with hidden trade-offs regarding your privacy and the quality of your care. I have chosen to be an out-of-network physician for three primary reasons:

    1. Privacy & Future Protection: To process a claim, insurance companies require a formal mental health diagnosis that becomes a permanent part of your medical record. This "paper trail" can sometimes impact your future eligibility or premium rates for life, long-term care, or disability insurance. By paying privately, your records stay here, and your data footprint remains minimal.

    2. Clinical Freedom: Insurance companies often limit the number of sessions or the type of treatment you can receive based on their internal "medical necessity" guidelines. I believe that your treatment plan—including how often we meet and for how long—should be decided by you and your doctor.

    3. Maximum Discretion: Using insurance allows third-party auditors to review your treatment plans and session notes to justify coverage. My private-pay model ensures that the details of your personal life remain strictly confidential within this office.

    Superbill - For those who wish to use their out-of-network benefits, I can provide a Superbill. Please note that submitting this to your insurance will require a formal diagnosis and may share certain data with your insurance provider

  • Ongoing care is available for select patients, when appropriate.

  • Medication recommendations are provided as part of the consultation. Ongoing prescribing depends on the treatment plan.

  • To maintain an environment of clinical excellence and logistical precision, my practice operates with a firm pre-payment requirement for all consultations.

    Payment & Confirmation

    As a boutique out-of-network practice, your session is a dedicated engagement of my clinical resources. Appointments are not confirmed and your time slot is not secured until full payment is received. Payment must be completed no later than 48 hours prior to your scheduled time; sessions without a cleared balance will be automatically released to our waitlist.

    Dr. Sadé D. Frazier, LLC accepts cash, all major credit cards, as well as specialized healthcare spending accounts (HSA & FSA).

    Cancellation & No-Show Policy

    By confirming your appointment, you are reserving an exclusive block of time.

    • Missed Sessions: Failure to attend your scheduled appointment (No-Show) will result in a charge of the full session fee.

    • Late Cancellations: Cancellations made within the 48-hour window are non-refundable and subject to the full session fee.

    This policy ensures that my focus remains entirely on your clinical outcomes. I thank you for respecting the professional nature of our time together.

Scope of Practice & Safety Notice

To ensure that every patient receives care that is both ethically sound and clinically responsible, I operate within a defined scope of practice. Certain clinical presentations require a higher level of care, intensive physical monitoring, or immediate multidisciplinary intervention that exceeds the safety capabilities of a Telehealth-only platform, such as:

  • Acute Safety Concerns: Individuals experiencing active thoughts, urges, or plans related to self-harm, suicide, or the intent to harm others. These situations require the immediate, in-person stabilization and the physical safety infrastructure of an emergency or residential setting.

  • Substance Use and Addiction: The management of active chemical dependency and withdrawal requires a level of in-person, multi-disciplinary oversight and physical medical monitoring that cannot be safely replicated via Telehealth.

  • Severe or High-Acuity Symptoms: Any condition or set of symptoms deemed by clinical assessment to be too severe for outpatient virtual management—including but not limited to acute psychosis, severe eating disorders requiring medical stabilization, or profound functional impairment—will be referred to a higher level of care.
